Step one: Check mandatory fields
First, ensure all mandatory fields are filled in. Mandatory fields are marked with a red asterisk * on the form. If you believe a mandatory field is unnecessary, please consult with a system admin.
Step two: Check sub-categories
Next, ensure that all sub-category fields are filled in. Sub-category fields appear on the form when specific options are selected. For example, if Other is selected from a drop-down, an additional field will appear for you to make further selections.
