ACC Mobile uses Single Sign-On (SSO) authentication through your organisation's Access Identity Provider. This means you'll use the same credentials as your desktop ACC account.

Multi-Organisation Users: If your account belongs to multiple organizations, you'll be prompted to select which organisation to access upon login. You can only access incident reporting in organisations where you have Incident v3 permissions enabled.

No Permissions: Users without any Incident v3 permissions will see a message indicating they don't have access to create or view incidents in the mobile app.

Permission-Based Features: The features available to you depend on your specific permissions on the web:

  • Users with MyIncidents_Create can create new incidents

  • Users with MyIncidents_CreateView can create and view their own incidents

  • Additional viewing and editing capabilities are granted based on your assigned permission levels

Important: If you don't have ACC credentials, contact your organisation's system administrator to request access.

Dashboard Overview

The Dashboard is your home screen in ACC Mobile. It provides quick access to create new incidents and view saved drafts.

Dashboard Options:

  • View and access incomplete incident reports saved as drafts

  • Start a new incident report from scratch with the Create New Incident button

  • Switch between Home and Incidents sections

  • Access additional settings and features via the three-dot menu

Creating a New Incident

Step 1: Initiate New Incident

From the Dashboard, tap the "Create New Incident" button

The New Incident screen will display with location and person selection

Step 2: Select Location and Primary Person

Select Location: Choose the care home or facility where the incident occurred

Primary Person Involved: Select the main person involved in the incident

Step 3: Complete Incident Reporting Sections

Once you've tapped "Add Incident", you'll see the incident progress tracker showing 0/6 Complete. The incident report consists of six main sections:

  • Incident Details

  • Type

  • People Involved

  • Description

  • Level

  • Support Plan

Each section must be completed to submit the incident. The progress indicator at the top of the screen shows your completion status.

Each section captures fundamental information about when and where the incident occurred and reflects the web version of the Incident Reporting form.

Tap "Save and Continue" to proceed to the next section.

Best Practice: Include specific details such as time, location, sequence of events, and immediate actions taken, people involved. Be objective and factual in your description.

Managing Draft Incidents

ACC Mobile automatically saves your progress as you complete each section. Draft incidents are stored locally and can be accessed anytime from the Dashboard.

Accessing Draft Incidents:

  • From the Dashboard, view the draft incidents area

  • Tap on any draft to resume working on it

  • The incident will open at the current status with progress indicator showing completion

Draft Incident Options:

  • Continue Editing: Complete remaining sections

  • View Progress: Check completion status via the circular progress indicator

  • Add Comments: Include additional notes or context

  • Link Related Incidents: Associate with other relevant incident reports

  • Delete Incident: Remove the draft if no longer needed (confirmation required)

Important: Drafts are stored locally on your device. If you uninstall the app or clear app data, your drafts will be lost. Complete and submit incidents promptly to ensure they're saved to the central system.

Adding Comments

Comments allow you to add supplementary information, notes, or follow-up details to an incident report.

  • From the incident view, tap "Comments"

  • If no comments exist, tap "Add Comment"

  • Enter a title for your comment

  • Enter the comment text in the comment field

  • Tap "Add Comment" to save

You can add multiple comments to track the ongoing status, actions taken, or additional observations related to the incident.

Linked Incidents

The Related Incidents feature allows you to Find incidents that are linked by person.

  • From the incident view, tap "Related Incidents"

  • Search for or select incidents from your system

  • Confirm the link to associate the incidents

This feature is particularly useful for tracking patterns, recurring issues, or multi-stage incidents that span multiple reports.

Deleting Incidents

Draft incidents can be deleted if they were created in error.

  • From the incident view, scroll to the bottom

  • Tap the red "Delete Incident" button

  • Confirm the deletion in the popup dialog

  • The incident will be permanently removed from your device

Deletion is permanent and cannot be undone. Only delete incidents that were created in error. Once an incident is submitted to the system, deletion options may be restricted.

Current Limitations

  • iPhone 12 Date Picker Crash (iOS 17) Users on iPhone 12 and iPhone 12 Pro running iOS 17 may experience an application crash when selecting the Date of Incident field in Incident Details. This issue is specific to these device models and iOS version. Workaround: Use an alternative iOS device or update to a newer iOS version if available.

  • iPhone SE 2020/2022 Section Access Users on iPhone SE (2020 and 2022 models) cannot access the "Incident reporting", "Comments", "Related Incidents", and "Download PDF" sections. This issue is under investigation. Workaround: Use an alternative iOS device to access these features.
