New: Incident V3 Analytics
We’re excited to introduce a completely new analytics dashboard for Incident V3, bringing incident data into Analytics for the first time. This purpose-built suite includes dedicated insights to help teams track, analyse, and respond to incidents more effectively.
Key insights include:
Incidents Last Month vs. 12-Month Trend – Spot emerging patterns
High-Risk Individuals – Identify those frequently involved in incidents
Incident Hotspots by Time – Understand when incidents are most likely to occur
Behavioural Incident Patterns – Reduce restrictive practices and improve support
Repeat Incidents – Flag unresolved issues
Staff Reporting Patterns – Promote a healthy reporting culture
Compliance Timeliness – Ensure timely submissions for CQC, RIDDOR, and safeguarding
Actions Taken & Follow-Up – Confirm incidents lead to meaningful change
To access the Incident V3 Dashboard:
Log into Access Evo and launch Analytics in the Product tab of the Evo button
Select Dashboards from the Analytics left hand menu and then "Incident V3 Dashboard"
Explore the purpose-built insights
Use drill-downs to investigate specific incidents, patterns, or trends
Note: If you’ve added new fields to your Incident V3 templates, a data refresh is required to reflect these changes in Analytics.
Data refresh is required when:
New fields are added to Incident V3 templates
Incidents are recorded using updated templates
You want new field data to appear in analytics
To perform a Data refresh:
Log into Access Evo and launch Data Engine in Product tab of the Evo button
Access Table&Views from the top right menu (by pressing three-line button)

Now Select “Refresh Schema” button
