A step-by-step guide to adding safeguarding capture to your Incident v3 form

Recording Safeguarding in Incident v3

Incident v3's form template is fully flexible, and while it doesn't include a safeguarding field by default, you can add one to your own version of the form in a few simple steps — and start reporting on safeguarding referrals straight away.

Customising your incident form

To capture safeguarding information, you'll need to add a section to your incident form template.

  1. Go to Incident Form Management in the admin area of Compliance.

  2. We'd recommend making a copy of the default template first, then building your safeguarding section on that copy rather than editing the original.

  3. Add a checkbox labelled "Is this a safeguarding concern?" (you're free to word this however suits your organisation).

  4. Use form conditioning so that ticking this checkbox reveals a set of additional fields — keeping the form simple until safeguarding details are actually needed.

Fields You Might Want to Capture

Once the checkbox is ticked, a good safeguarding section may typically capture:

  • Date of referral

  • Type of referral (e.g. Physical)

  • Reason for referral

  • Status of safeguarding

  • And more

You can also add a Documents area so supporting evidence can be attached directly to the referral.

Your safeguarding section can also be added as its own subsection in Incident Review or elsewhere, sitting alongside steps like Investigation and External Notification.

Before you publish, it's worth checking the form for any additional fields or dependencies your team may already have configured around the safeguarding checkbox, so your new section fits in cleanly rather than duplicating existing logic.

Once you're happy with the layout, publish the form to the appropriate locations. Publishing automatically replaces the previous version, so any incidents raised afterwards will use your updated template.

Reporting on safeguarding incidents

Once published, safeguarding incidents can be submitted or completed via the ACC Incident v3 mobile app or the web

Because the safeguarding fields are part of the incident form itself, they'll flow through to both Access Data Engine and Analytics once your section is published and you've configure it to show. That means the additional fields sit alongside the rest of your incident data, ready to use whether you're pulling a quick export or building out your own custom reports.
