What is Person Centred 360?
Person Centred 360 gives you a single joined-up view of any person you support. Instead of jumping between different modules to piece together a picture of a supported person, everything you need is in one place: incidents, audits, complaints, compliments, and actions all on one screen.
The dashboard is powered by an AI Evo Agent that automatically generates summaries, surfaces patterns in the data, and helps you spot trends.
Where to find it: Person Centred 360 is available as a EVO Navigator widget as a Quick Section or a Quick Action.
Finding a Person
When you open Person Centred 360, you will see a search field at the top of the screen. Type the person's name or their unique identifier to find them. Select their name from the dropdown and their full 360 profile will load.
The Person Header
At the top of the profile you will see a summary of the person's key details at a glance.
What you see | What it means |
Home | The care home or location where the person is receiving care |
Room | The room the person has been assigned to within the service |
Age | Calculated automatically from the person's date of birth |
Risk tier | Based on the Risk Insights score, which measures how well the person's record is kept up to date (Low, Medium, or High) |
Admitted | The date the person was admitted into care |
At a Glance: The Stat Tiles
Below the person header you will find a row of tiles that give you an instant count of key activity:
Incidents - number of incidents as the primary person involved
Complaints - number of complaints made by the person receiving care
Incidents without regulatory notifications - incidents where a regulatory notification may be required but has not yet been sent
Audits - audits due or overdue
Repeated incident types - patterns of the same type of incident occurring more than once (for example, repeated falls)
Incidents without linked actions - incidents that have been recorded but have no follow-up action linked to them
These tiles are designed to help you catch compliance gaps early. If any tile shows a number greater than zero, it is worth reviewing those records.
AI Risk Summary
The AI Risk Summary is a plain-language narrative generated by the Evo Agent. It pulls together the person's recent activity and risk data into a short, readable summary.
You can regenerate the summary at any time to reflect the latest data.
The AI Risk Summary is a decision-support tool. Always use your professional judgement alongside it.
AI Complaints Summary
Similar to the AI Risk Summary, the AI Complaints Summary gives you a short narrative overview of the person's complaint history. It highlights key themes so you can quickly understand the complaints picture.
AI Risk Insight Score
The AI Risk Insight Score gives you a calculated rating of how much attention a person's record needs, based on their incident history, audit outcomes, complaints, and other compliance data. The score is broken down into contributing factors so you can see exactly what is driving it.
A radar chart shows how that attention is distributed across different areas such as incidents, safeguarding, audits, and complaints. This makes it easy to see at a glance where the record needs the most focus.
Recent Incidents
The Recent Incidents card lists the person's most recent incidents, showing the incident type, severity, and current status. Each entry links directly to the full incident record in Access Care Compliance.
Audit Status
The Audit Status section lists all audits associated with the person, showing whether each one is due, in progress, overdue, or completed. Each audit links directly to the record in Access Care Compliance so you can open it without leaving the dashboard.
Staff Involvement Patterns
The Staff Involvement card shows which staff members appear most frequently in the person's incident and activity records.
Activity Timeline
The Activity Timeline shows a chronological feed of all activity recorded for the person such as incidents, audits, complaints, compliments, and actions — sorted with the most recent at the top.
Frequently Asked Questions
Can I edit records from within Person Centred 360?
No. Person Centred 360 is a read-only dashboard. To edit or update a record, click the link on the relevant item and it will take you directly to that record in Access Care Compliance.
How often is the data updated?
The dashboard reflects the data currently held in Access Care Compliance. The AI summaries can be regenerated at any time to pick up the latest changes.
What does it mean if the AI Risk Insight Score shows Low Risk for someone with no recorded incidents?
A Low Risk score for someone with no incidents, audits, or assigned risks is expected behaviour. It reflects the absence of recorded risk factors on their record, not a confirmed assessment of the person themselves, a low score means there's little in the system to flag, not that they've been reviewed and cleared.
